Laparoscopic management of ovarian tumors

Rezumat: For the last decades laparoscopy proved itself as valuable tool for gynecological pathology management. The objective of this paper is to highlight the advantages of laparoscopy both in the diagnosis as for therapeutic management of ovarian tumors. The choice of surgical approach depends on the suspicion of malignancy, the surgeon and patient options. According to current literature, it is the first line approach for ovarian pathology with low risk of malignancy after applying ultrasound and biochemical scores. Initial exploratory laparoscopic approach is justified also in menopause when there is an increased necessity for surgery in order to rule out malignancy. A benign anexial mass or very likely to be benign can be treated by laparoscopic cystectomy before premenopausal and through adnexectomy postmenopausal time. Whenever malignancy is very likely, a laparotomy is mandatory. In conclusion, laparoscopic surgery is the first line choice for ovarian mass, due to undeniable advantages: minimal postoperative pain, quick recovery, low infection risk, reduced postoperative complications and prognosis of ovarian optimally. Rigorous and accurate selection of cases leads to success through laparoscopic interventions.
