Doppler velocimetry of uterine leiomyomas – indicator of further development

Rezumat: Doppler velocimetry was carried out in 57 pre-menopausal and 6 post-menopausal women. Leiomyoma volume was measured 2 times (first visit and 6months follow-up); peritumoral and intratumoral vessels blood flow was assesd using peak systolic (PSV) and end-diastolic (EDV) velocities and resistance index (RI). Increased peripheral and central vascularisation findings (with high sensitivity -98% and specificity-87%) are directly correlated with fibroids growth (p<0.005). The mean RI measured of the periphery of the leiomyomas was 0.56 (p<0.05) and the mean PI was 0.89 (p<0.05). Significant correlations were assessed between RI of analysed vessels and myomatous tissue with high cell proliferation rate-seems to have higher vascular resistance. Marked reduction of RI and PI and increased PSV were found in leiomyoma cases which showed large size and/or necrotic, degenerative and inflammatory changes. The RI decreased and the PSV increased with increasing fibroid size.
